2 The hospital discharge planning team or agency works with the IF Recuperative Care referral team to decide the patient's length of stay.
3 Once the referral has been approved, the hospital transports the patient to the IF Recuperative Care facility.
4 The patient is housed, provided daily meals, and supervised as they recover from injury or illness at IF Recuperative Care.
5 The patient is connected to supportive services and housing options to facilitate long-term stabilization.
6 IF Recuperative Care manages discharge of the patient from the facility to successfully transition them back into the community.
